Health Appraisal form

Students enrolled in Our Shepherd Lutheran Preschool must have a Health Appraisal form on file prior to the first day of school. A valid Health Appraisal form must be signed no earlier than May, 2010 for the 2011-2012 school year.

The Health Appraisal form must be complete (Personal Information, Section I - Health History, Section II - Immunizations, Section III - Physical Exam, and Section IV - Recommendations).
